Quest help - Angel of Torment

My Sorc (lvl 528) was quested for an Angel of Torment.  From what I have seen on the forum, I should be able to find this on 15Uber in a place called the Temple.  However, in my couple trips down to 15Uber, I have not found the temple. Here are a few questions:

• Is this one of those off map areas that has to be teleported to?
• If so, then which tp pad to I take to get to it?
• Does anyone have maps to the temple available?
• Last one: What is the best way to hit them so I don't die horribly?
